What is a PHR?

Personal Health Records (PHRs) are electronic applications through which individuals can enter, modify, maintain and manage their health information in a secure environment at the convenience of their computer.

A PHR contains information about your medical health history, family health history, emergency contacts, physician information, insurance information, vision, dental, immunizations, hospitalizations, surgeries, allergies, infectious diseases, diagnostics/lab information, medical devices and physical/occupational therapies.

Why Should One Complete a PHR?

PHR provides a comprehensive approach to represent your health and can be customized to include your medical health history, family health history, prescription related information, and pathological tests you have undergone, surgeries/procedures and other health related information.

This report can be printed and carried along with you to your doctor or dentist, for a second opinion. We suggest carrying this with you during while traveling in case of any unforeseen medical emergency.

How long would it take to complete a PHR?

Approximately 10 minutes.

Just remember that you can always return to this record to update your information at any time in case you are not able to complete all the information in one shot.

Is there anything else I should know about completing a PHR?

While completing your health information, accuracy and openness is vital for a proper comprehension of your health by your health care professional. The only way that he/she can provide you with the most effective and useful treatment is if the information stated on your record is complete and accurate in all respects.

Who can Benefit the most from a PHR?

  • If you are the center of support of your family, this is a convenient way to manage the health records of the whole family, whether it be your parents, spouse and/or children.
  • Patients of chronic health conditions need to keep a track of all treatments and care from multiple health professionals to ensure any forthcoming treatment plan.
  • In case of a provider of aging adults, this provides a concise method of maintaining the health and medical record.
  • Any person with special dietary needs.
  • Any child with special needs.

How often should I update my Personal Health Record?

We recommend that you update your records at least once in every Six Months. However you may update this any time there are changes to your Health Status.
